Asset integrity management is an essential part of the Mechanical Integrity element in the Process Safety Management framework. Programs such as risk-based inspection evaluate operational risks and establishes inspection plans to prevent unplanned facility failures. However, an effective Asset Integrity practice should also consider residual risks originating from the manufacturing and fabrication phase. Neglecting these risks can lead to unexpected failures during operations. A recent incident arising from improper heat treatment of stainless steel components in high-temperature refinery service highlighted the need for enhanced integrity measures. In response to this, a materials assessment was initiated, analyzing data from the equipment quality dossier. Gaps in manufacturing and fabrication processes were identified, and mitigated by a customized inspection program. This paper discusses crucial factors during manufacturing and fabrication that may cause degradation of material properties, resulting in premature failure. Additionally, it explores safety risks and other identified gaps from the study, along with the corresponding mitigation measures.

Digitalization is the use of digital technologies and digitally-enabled approaches to enable or improve business models and processes. The use of remote tablets to capture inspection data in real-time and the use of streaming video are key examples of digitalization. The addition of sensors to monitor the health of an asset is a component of enabling a digital twin concept, which is enabled with historical, real-time and forecast data. This document presents current use cases on the use of digital twin platforms to visualize assets and provide context for operational data. The use of hyperspectral sensors to help enhance the digital twin environment adds lifecycle features that enable more efficient condition monitoring. An overview of the BIM (Building Information Modeling) service supporting the digital lifecycle of 3-D/2-D model integrations will be discussed. This will also include the use of AR/VR devices including Microsoft’s Hololens and the delivery of ‘model load packages’, which allows the user to view the various models required to integrate with Hololen’s features. The combined use of augmented inspection reporting and corrective maintenance instructions allows for a streamlined inspection approach through the use of in situ sensors and mobile inspection campaigns.

Optimizing Equipment Availability through Structured Engineering Assessment

The recognized international codes, such as ASME [1], API[2], BS[3] provide design and fabrication guidelines for various equipment used in industries viz. pressure vessels, piping and storage tanks. However, the fabrication standards do not address acceptability of the equipment on detection of any deterioration; once put in to the operation.
The engineering assessment becomes essential when flaws such as “leakage”, ‘metal loss’ or ‘linear indications’ are detected during routine inspections. Sometimes, observed damages or flaws, do not meet the definition criteria as per assessment steps given in the international standards of fitness for service. In order to asses such damages, a structured engineering approach based on experience and judgment would help in such situations.
Describing the structured engineering assessment approach, this paper discusses a case study on typical ‘hydrogenation reactor’ that displayed micro-level leakage. Until 2015, the reactor often leaked during service prior to the assessment. This approach using various NDT methods, microstructural examination and structured engineering analysis identified the problem of dilution of base metal with welding that was leading to inter dendritic corrosion at micro level. The structured engineering analysis paved the forward path by which the reactor could be operated without any unscheduled outages since then after providing improved welding procedure.
